Abstract
Atopijski dermatitis je recidivirajuća upalna bolest kože složene etiopatogeneze čiji su karakteristični simptomi svrbež, suhoća kože i crvenilo u području vrata, obraza, gornjih i donjih ekstremiteta. S obzirom na sve veći broj oboljelih i sve teže kliničke slike, javlja se potreba za pronalaskom novih dugoročnih terapija. Biološki lijekovi kao novija terapijska opcija održavaju bolest u stadiju remisije. Etiopatogeneza atopijskog dermatitisa uključuje mnogobrojne imunološke mehanizme i kaskadne reakcije pa se ciljne molekule bioloških lijekova razlikuju s obzirom na mjesto ili signalni put djelovanja. Prvi odobreni biološki lijek za liječenje atopijskog dermatitisa je dupilumab koji djeluje inhibitorno na sinergistički put interleukina 4 (IL-4) i interleukina 13 (IL-13) čime onemogućuje daljnju alergijsku reakciju. Fezakinumab je inhibitor interleukina 22 (IL-22) koji je uključen u upalni proces i smanjuje vrijednosti ostalih imunoloških medijatora – Th2, Th22, Th17 i Th1. Upadacitinib je inhibitor JAK kinaze koji suprimira više signalnih puteva, za razliku od inhibitora IL-4/IL-13 ili inhibitora IL-22 koji imaju ciljano djelovanje.
Abstract (english)
Atopic dermatitis is a recurring inflammatory skin disease with complex etiopathogenesis whose characteristic symptoms are itching, dry skin and redness in the neck, cheeks, upper and lower extremities. Considering the increasing number of patients and the increasingly difficult clinical pictures, there is a need to find new long-term therapies. Biological drugs are a relatively new therapeutic option that keeps the disease in remission. The etiopathogenesis of atopic dermatitis involves numerous immune mechanisms and cascade reactions, so the target molecules of biological drugs differ with regard to the place or signaling pathway of action. The first approved biological drug for the treatment of atopic dermatitis is dupilumab, which has an inhibitory effect on the synergistic pathway of interleukin 4 (IL-4) and interleukin 13 (IL-13), thereby preventing further allergic reactions. Fezakinumab is an inhibitor of interleukin 22 (IL-22), which is involved in the inflammatory process and reduces the values of other immune mediators – Th2, Th22, Th17 and Th1. Upadacitinib is a JAK kinase inhibitor that works by suppressing multiple signaling pathways, unlike IL-4/IL-13 inhibitors or IL-22 inhibitors that have a targeted action.
