Abstract
Svrha istraživanja bila je odrediti opskrbljenost selenijem (Se) stanovništva područja u kojem se javlja endemska nefropatija (EN), i temeljem toga procjeniti njegovu ulogu u nastanku ove bolesti. Provedene su analize namirnica s endemskog i kontrolnog (u kojem nema pojave ove bolesti) područja, te utvrđena potrošnja istih korištenjem validiranog upitnika učestalosti namirnica. Prehrambene navike su ispitane u uzorku kontrolne populacije, te u tri podskupine endemske populacije: članovi obitelji u kojoj je bilo pojave EN, ispitanici u čijoj obitelji nije bilo pojave EN, te skupina pacijenata od ove bolesti. U skupini pacijenata su utvrđene značajke prehrane i unos Se koji mogu utjecati na prognozu bolesti. Nema većih razlika između kontrolne i endemske populacije u unosu osnovnih hranjivih tvari. Identični udjeli hranjivih tvari članova obitelji i pacijenata upućuju na moguću etiološku ulogu prehrambenih navika u razvoju EN i tumora mokraćnog trakta. Endemska populacija ima prosječno viši unos Se od kontrolne, pri čemu najveći dio ispitanika ima dnevni unos viši od preporučenih vrijednosti. Nepostojanje deficita Se u endemskoj populaciji ipak ne isključuje značenje ovog elementa u etiologiji EN dok se ne odredi izloženost pravom uzročniku bolesti.
Abstract (english)
The aim was to estimate provision of selenium (Se) to the population of area affected with endemic nephropathy (EN), and assess this as possible risk factor in the etiology of the disease. Analyses of foods from both endemic and control (with no occurrence of EN) areas, and food consumption data using a validated food frequency questionnaire, were performed. There were three subgroups of endemic population: members of families in which EN occurred, subjects with no familial occurrence of this disease, and also a group of EN patients. In the group of patients, some features of nutrition and Se intake could influence the prognosis of the disease. There are no major differences between control and endemic populations in intakes of elementary nutrients. Identical ratios between nutrients in the group of patients and the group of patients’ family members could be an indication of etiological role of nutritional habits in EN and urinary tract tumours. Endemic population has a higher average Se intake compared to control population, with most subjects having dietary Se intake above recommended values. However, the lack of Se deficit in endemic population does not preclude its etiological role in EN until exposure to actual causative agents is known.
