Abstract Međunarodno udruženje za proučavanje boli (eng. The International Association for the Study of Pain, IASP) opisalo je sindrom pekućih usta kao kronično stanje okarakterizirano osjećajem pečenja usne šupljine čiji uzrok nije moguće pronaći. Intenzivan osjećaj pečenja odnosno žarenja i boli usne šupljine najizraženiji je na prednjem dijelu jezika, no može zahvatiti bilo koji dio sluznice usne šupljine. Javlja se spontano, obično bilateralno odnosno simetrično, a prisutan je tokom cijelog dana. Pečenje i bol intenziviraju se tokom dana, slabije su izraženi neposredno nakon buđenja, u jutarnjim satima, a ublažavaju se i nakon obroka ili konzumacije pića. Kao popratni simptomi se kod pacijenata javlja i osjećaj suhoće usta kao i promjena osjeta okusa što dodatno narušava kvalitetu njihova života. Unatoč navedenim simptomima na sluznici nije moguće vidjeti nikakve promjene ili lezije, ona je klinički zdrava. Najčešće se javlja u žena i to u postmenopauzalnom razdoblju. Incidencija pojave sindroma pekućih usta
raste s brojem godina, rijetko ga nalazimo u mlađih osoba. Mehanizam nastanka i dalje nije dovoljno istražen no znamo da se radi o neuropatskoj boli koja može biti potaknuta brojnim lokalnim, sistemskim i psihološkim predisponirajućim čimbenicima. Dijagnostika se temelji na detaljnom kliničkom pregledu i opće medicinskoj odnosno stomatološkoj anamnezi uz isključivanje drugih medicinskih stanja sličnih simptoma koji bi mogli biti potencijalna dijagnoza. Važno je istaknuti da je za dijagnozu potreban nalaz klinički zdrave sluznice, bez vidljivih lezija. Kao terapija sindroma pekućih usta na raspolaganju nam je farmakološka i
nefarmakološka terapija. Izbor same terapije ovisit će o iskustvu kliničara i samom zdravstvenom stanju pacijenta jer ne postoji univerzalan lijek za koji možemo tvrditi da će sigurno pomoći u terapiji sindroma. Dakle terapija sindroma pekućih usta individualna je. Kod nekih pacijenata nažalost neće biti učinkovita, a kod nekih se simptomi sindroma mogu sami povući. Sindrom pekućih usta je specifična dijagnoza zbog još uvijek nedovoljno podataka o točnom mehanizmu nastanka, svim uzročnicima koji mogu inicirati njegov nastanak, ali i samom načinu liječenja.
Abstract (english) The International Association for the Study of Pain described burning mouth syndrome as a chronic condition characterized by a burning sensation of oral mucosa for which no cause can be found. Intensive burning sensation and pain are usually appear in the anterior part of the tongue, but other oral cavity sites may also be affected. The burning sensation starts
spontaneously, is usually symmetrical and present every day for most of the time. The sensation reaches its maximum during the day and its minimum or iscompletely absent early in the morning or during mealtimes. The burning sensation and pain are often accompanied by the dryness of the mouth and reduced taste intensity. Despite all these symptoms, the oral mucosa remains normal. Most of the affected patients are postmenopausal women. The frequency of burning mouth syndrome increases with age and is an extremely rare condition in young patients. The etiology of burning mouth syndrome is still poorly understood. Still, we know that the syndrome has the characteristics of neuropathic pain that can be caused by numerous local, systemic and psychological factors. A detailed clinical examination, next to thorough anamnesis and the exclution of various diseases with the same symptomatology are necessary for a correct diagnosis of this syndrome. A diagnosis can be made only in cases where the oral mucosa is clinically normal. Burning mouth syndrome can be managed by pharmacological or by nonpharmacological methods. There is no standard treatment since it is empirical and primarily based on personal and expert opinion. Management is purely symptomatic, and therefore expectations of the treatment's result should not be unrealistic. Burning mouth syndrome is a complex condition with little knowledge of etiology, causative factors or satisfactory treatment.
