Abstract Strabizam je relativno učestali poremećaj očiju s neusklađenosti optičkih osi, koji rezultira abnormalnom binokularnom interakcijom te razvojem slabovidnosti. Unatoč napretku tehnologije posljednjih godina, temelj dijagnosticiranja strabizma te fizikalnog pregleda
postavljen je još potkraj 19., odnosno početkom 20. stoljeća, a zasniva se na stručnosti i uvježbanosti liječnika subspecijalista dječje oftalmologije u postavljanju dijagnoze. U ovom radu predstavljen je suvremeni, računalno utemeljeni uređaj za praćenje pokreta oka u dijagnosticiranju strabizma. Rezultati mjerenja uređajem za praćenje pokreta oka kao i rezultati standardnog oftalmološkog pregleda su detaljno analizirani na ukupno 181 ispitaniku. Za analizu je korištena ROC (od engl. Receiver Operating Characteristic) krivulja s ocjenom valjanosti dijagnostičkih uređaja i testa prema ostvarenim vrijednostima površina ispod krivulje
AUC (od engl. Area Under Curve), koji se uobičajeno rabe za ocjenjivanje senzitivnosti i specifičnosti pojedinoga dijagnostičkog postupka ili uređaja. Evaluacija je pokazala kako je metoda dovoljno osjetljiva i specifična za dijagnosticiranje strabizma bez potrebe za usko specijaliziranim ispitivačem, te s mogućnošću primjene u probiru strabizma.
Abstract (english) Introduction: Strabismus is a common disorder in which the eyes are not aligned with their optical axis, resulting in an abnormal binocular interaction that leads to amblyopia. Accordingly, early detection and diagnosis are mandatory. Despite technology developments and constant knowledge growth, the foundations of the physiology and diagnosis of strabismus
were set in the 19th and early 20th century and have not changed since then. This paper presents a novel eye-tracking method for strabismus diagnosis, developed as a joint venture between the Ophthalmology Department of the Sestre Milosrdnice University Hospital Centre and the Ruđer Bošković Institute.
Patients and Methods: This study was conducted at the Ophthalmology Department, Sestre Milosrdnice University Hospital Centre in Zagreb, Croatia. The study included 181 participants who underwent complete ophthalmological examination, including strabology status, and were tested using a computer-based eye-tracking device and optical occludor to fashion the cover/uncover test principle. Patients were divided into three groups based on the results of the ophthalmological examination. The control group consisted of 61 healthy subjects without strabismus, the manifest strabismus group included 59 subjects and the latent strabismus group included 61 subjects. Subjects younger than 4 years or older than 60 years, and those with previous ophthalmic surgeries, diseases, or any conditions affecting eye motility were excluded from the study.
Results: The results of the complete ophthalmological examination and the novel, computerbased eye-tracking apparatus were analysed using the WWII-based Receiver operating characteristic curve (ROC) analysis, which uses the values of ROC space or the area under the curve (AUC) to grade methods or instruments. In our case, it justified the known facts regarding standardised ophthalmological examination, and also highlighted unknown facts concerning the eye-tracking apparatus. Standard ophthalmological examination was regarded as excellent according to the ROC analysis in cover test variants and the Hirschberg test. The eye-tracking apparatus confirmed the results of the standard ophthalmological examination. According to the AUC values in the ROC analysis, eye-tracking was regarded as a good tool for most of the
computed variables, and excellent primarily when accompanied with an optical occludor.
Conclusion: The evaluation showed that this method has high sensitivity and specificity in detecting manifest and latent strabismus without the need for a skilled examiner, and as such is suitable for population screening.
