The selection of implant abutment material can greatly influence the esthetics of dental implant therapy. Implant abutments serve as the link between the implant and the final restoration. The purpose of this review was to evaluate the influence of implant abutment material choice on the esthetics of dental implant therapy. Three common abutment materials include titanium, zirconia, and custom-made ceramic. While titanium abutments offer strength and stability, their metal colour can be visible through the gingiva and detract from esthetics. Zirconia abutments offer improved esthetics with a white colour that mimics natural teeth, but their brittleness may limit their use. Custom-made ceramic abutments offer optimal esthetics with a translucent quality that closely resembles natural teeth and the ability to match surrounding tooth colour. In addition to the esthetics of the abutment material itself, the final restoration material choice can also impact the esthetics of the implant therapy. Materials such as composite resins, porcelain veneers, and ceramics can be selected to enhance the overall esthetics of the restoration. In conclusion, the selection of implant abutment material is an important factor in the esthetics of dental implant therapy. Custom-made ceramic abutments offer the best esthetics due to their translucency and ability to match surrounding tooth colour, while titanium abutments may detract from esthetics due to their metal colour. The proper selection of abutment and final restoration materials is essential for achieving optimal esthetics in dental implant therapy.
