Abstract
Od početka upotrebe 1970-ih, intrakoštani oralni implantati postali su integralni dio rekonstruktivne stomatologije. U početku se terapija implantatima primjenjivala samo za potpuno bezube pacijente. Posljednjih godina, ipak, pacijenti s djelomično ozubljenim čeljustima postaju kandidati za postavu implantata. Opravdanost primjene dentalnih implantata podupiru mnogobrojne studije koje govore o uspjehu i dugotrajnosti oseointegracije implantata. U manjem broju slučajeva mogu nastati komplikacije i s tim u vezi prijevremeno rasklimavanje ili gubitak implantata. Preostali zubi s parodontnom bolesšću mogu biti izvor za naseljavanje implantata oportunističkim parodontnim patogenima, što rezultira patološkim promjenama periimplantatnog tkiva. Upalne promjene ograničene na sluznicu oko implantata nazivaju se periimplantatni mukozitis, a uznapredovali gubitak periimplantatne kosti zajedno s upalnim promjenama mekih tkiva označava se kao periimplantitis. Periimplantatni mukozitis u pravilu se liječi konzervativno, antibiotskom terapijom. Kod periimplantitisa dodatno se primjenjuju i kirurški postupci, u smislu regenerativne terapije. Kumulativna interceptivna potporna terapija (CIST) uključuje seriju od četiriju protokola koji se rabe nakon što se završe ispitivanje i dijagnostički postupci. Pacijent s implantatom mora se uvijek uključiti u program potporne terapije, koji uključuje ponovne posjete u pravilnim intervalima. Svaki posjet mora početi ispitivanjem kako bi se ustanovilo jesu li mjesta implantata zdrava ili pokazuju znakove upale. Cilj je potporne terapije održati gingivalno i parodontno zdravlje uspostavljeno aktivnom terapijom.
Abstract (english)
Since beginning of use in 1970's, intra-oral implants became part of reconstructive dentistry. In the beginning implant therapy was used only for toothless patients. However, recently, patients with partial tooth loss became candidates for implants. We can find justification for using dental implants in many studies of success and long-term osseointegration of implants. Complications could occur in smaller number of cases, related to non-stable implant or implant loss. Remaining teeth, infected with some periodontal disease, could be the source for implant infection with opportunist periodontal pathogens, which can result in pathological changes in peri-implant tissue. Infectious changes, limited on the area of mucosa around implant, are called peri-implant mucositis. Therefore advanced loss of peri-implant bone along with infectious changes of soft tissues is marked as periimplantitis. In general peri-implant mucositis can be treated conservatively, with antibiotic treatment. Regarding to regenerative therapy, surgical procedures can be applied. Cumulative interceptive support therapy (CIST) includes series of four protocols which are used after the testings and diagnostics procedures. Implant patient has to be involved in program of support therapy, that includes check-up appointments in regular intervals. Each appointment has to be started with acknowledge that implant surrounding is healthy and shows no sign of infection. Final mission of support therapy is gingival and periodontal health ensured by active therapy.
