Abstract Podizanje dna maksilarnog sinusa kirurška je metoda koja se primjenjuje kod opsežne resorpcije alveolarnoga grebena stražnjeg dijela gornje čeljusti. Resorpcija kosti je posljedica pneumatizacije maksilarnog sinusa najčešće nakon gubitka molara, rjeđe premolara u gornjoj čeljusti. Prije zahvata potrebno je uzeti anamnestičke podatke te napraviti klinički i radiološki pregled kako bi se izbjegle moguće komplikacije. Radiološki se određuje anatomski oblik sinusa, visina koštanoga grebena ili bilo kakve patološke promjene. Kod podizanja dna maksilanog sinusa postoje određene kontraindikacije, kao patološki promijenjena sluznica sinusa, bilo u obliku sinusitisa, cista ili tumorske mase.Podizanjem dna maksilarnog sinusa postiže se povećanje grebena u gornjoj čeljusti, čime se omogućuje ugradnja dentalnih implantata. Odgođena tehnika ugradnje primjenjuje se kod velikoga gubitka kosti, dok se imedijatna koristi kada ima više od 4 mm visine koštanoga grebena. Opisane su različite tehnike podizanja dna maksilarnog sinusa. Vanjska ili lateralna tehnika rabi se ako je preostalo manje od 6 mm visine koštanoga grebena, dok se unutarnja ili transkrestalna tehnika primjenjuje kada je preostalo više od 7 mm visine koštanoga grebena. Kako bismo omogućili zadovoljavajuću stabilnost implantata, osim podizanja dna maksilarnog sinusa potrebno je nadoknaditi gubitak kosti augmentatvnim materijalima, koji mogu biti autogeni, alogeni, ksenogeni ili aloplastični.Kao i kod svakoga operativnog zahvata postoji mogućnost nastanka komplikacija koje mogu nastati tijekom ili nakon zahvata. Najčešća komplikacija tijekom zahvata je ruptura Schneiderove membrane i obilno krvarenje, dok je pojava sinusitisa najčešća komplikacija nakon zahvata.Osnovna je svrha podizanja dna maksilarnog sinusa mogućnost postavljanja implantata, izrada protetskog nadomjestka te konačno vraćanje estetike i funkcije kod pacijenta.
Abstract (english) Maxillary sinus lift is a surgical procedure applied to extensive resorption of the alveolar ridge in a distal region of the upper jaw. Bone resorption is a result of pneumatization of the maxillary sinus usually after the loss of molars, less frequently premolars in the upper jaw. Before the procedure it is necessary to take a medical history and do a clinical and radiological examination in order to avoid possible complications. Radiological evaluation determines anatomical shape of the sinuses, the height of the bone ridge or any pathological changes. Sinus lift procedure has certain contraindications, as there are pathological changes of mucous membranes of the sinuses, whether in the form of sinusitis, cysts or a tumor mass.A sinus lift procedure is achieved by increasing the ridge in the upper jaw, which enables the placement of dental implants. Delayed implant placement is applied to large bone losses, while the immediate implant placement is used when there are more than 4 mm in height of the bone ridge. There are many sinus lift techniques. The lateral sinus lift technique is used if there are less than 6 mm in height of the bone ridge, while the internal or transcrestal technique is applied when there is more than 7 mm in height of the bone ridge. When sinus lift is necessary, the gap between bone and membrane is also augmented with various bone materials, which can be autogenous, allogeneic, xenogeneic or allografts, in order to ensure satisfactory implant stability.As with any surgery, there are potential complications that can occur during or after the procedure. The most common complication during the procedure is a rupture of Schneider's membrane and profuse bleeding while the occurrence of sinusitis, the most common complication after the procedure.The primary purpose of maxillary sinus lift is the possibility of placing dental implants and prostetic restoration as well as the functional and aesthetic restoration of teeth.
