Abstract | Pulpne bolesti klinički možemo klasificirati kao reverzibilni, ireverzibilni pulpitis, nekrozu, te ostale bolesti kao što su kalcifikacije i interna resorpcija. Patološke promjene u pulpi rezultat su brojnih podražaja koji dovode do reakcije pulpe obrambenim procesom, upalom. Ako se proces pravodobno ne dijagnosticira i liječi, kao posljedica nemogućnosti širenja pulpe i nedostatka kolateralne cirkulacije dolazi do nekroze, a onda i periradikularne patoze. Glavni simptom je pulpna bol koja je duboka, visceralna, intenzivna bol, duljeg trajanja, može nastati spontano i katkad biti iradirajuća. Pulpna bol je difuzna i teško ju je lokalizirati, jer pulpa sadrži samo vlakna za bol. Dijagnostika uključuje brojne postupke za prikupljanje podataka, čija nam interpretacija omogućava postavljanje radne dijagnoze. To su: dentalna i medicinska anamneza, ekstraoralna i intraoralna ispitivanja, koja uključuju kliničke testove perkusije, palpacije, testove vitaliteta (termalne i električne), parodontni test, specijalni testovi, te RTG dijagnostika. |
Abstract (english) | We can clinically classify pulpal diseases as reversible, ireversible pulpitis, necrosis and other diseases, such as calcifications and internal resorption. Pathological changes of pulp tissue are the result of numerous iritations, which lead to a defensive process, inflammation. If the process is not diagnosed and cured in time, as the result of uncapabillity of pulpal spreading and lack of the collateral circulation, necrosis follows with periradicular pathosis after. The main symptom is pulpal pain, which is deep, visceral, intensive pain that lasts long, it can arise spontaneusly and sometimes iradiate. Pulpal pain is diffuse and presents problem in it' s localization, because pulp only contains fibers for pain. Process of diagnosing includes many procedures for assembling all necessary data, which will be interpreted to make final diagnosis. Those are: dental and medical
history, extraoral and intraoral examination, which includes clinical tests of percussion, palpation, pulp vitality tests (thermal and electrical), periodontal test, special tests and RTG interpretation. |
