Abstract
Resorpcija alveolarnog grebena bezube gornje čeljusti često je rezultat ekstrakcije zuba i jake pneumatizacije maksilarnog sinusa. Kao takva, predstavlja veliki izazov protetičkoj rekonstrukciji denticije. Prvi su koraci terapije dijagnostika i planiranje. Mnogi se kliničari odlučuju za tehniku augmentacije kosti i podizanja dna maksilarnog sinusa, kako bi nadoknadili izgubljenu kost i pripremili ju za kirurški zahvat implantacije. U svrhu augmentacije mogu se koristiti autogeni ili alogeni transplantati te ksenotransplantat i aloplastični koštani nadomjestci. Tehnika podizanja dna sinusa ima više metoda, izbor ovisi o visini alveolarne kosti gornje čeljusti. Alternativa konvencionalnim tehnikama upotreba je zigomatičnih implantata. Prvobitno se ova vrsta implantata koristila kod pacijenata s velikim defektima maksile uzrokovanih kongenitalnim defektima, resekcijom tumorskih lezija ili traumom. Indikacija za implantaciju zigomatičnih implantata proširila se na rehabilitaciju umjereno do teško atrofirane maksile.
Abstract (english)
Alveolar ridge resorption of edentulous upper jaw is often the result of tooth extraction and substantial pneumatization of the maxillary sinus. As such it represents a major challenge for prosthetic reconstruction of dentition. The first step of therapy is diagnosis and planning. Many dental clinicians opt for bone augmentation technique and elevation of maxillary sinus floor to compensate for the missing bone and prepare it for implant surgery. For the purpose of augmentation one can use autogenic or allogeneic transplants and xenotransplants and alloplastic bone substitutes. The technique of sinus floor elevation has several methods, the choice depending upon the height of the upper jaw alveolar bone. An alternative to conventional techniques is the use of zygomatic implants. Initially this type of implants was used in patients with substantial maxilla defects caused by congenital defects and for tumor lesion resection or trauma. The indication for implantation of zygomatic implants has extended to rehabilitation of moderately to severely atrophied maxilla.
