Periodontitis is one of the most common inflammatory diseases that destroys periodontal structure of the tooth. The objectives of the periodontal surgical therapy are prevention of further disease progression, and regeneration of the supportive tissue. The use of enamel matrix proteins has achieved satisfactory results in regenerative periodontal surgery. During healing, such proteins stimulate the formation of cement with inserted collagen fibres by imitating the conditions that occurred during the development of teeth. Active components that have regenerative capability are amelogenins and neamelogens present in a mixture of enamel matrix protein. Emdogain® is a commercially available product containing porcine germinal matrix derivatives and has been present on the market for over 20 years. Use of Emdogain® is a safe procedure that results in regeneration of the tissue. It has been proved that, if administered alone, it produces the same effect as the GTR method, whereas no significant advantage has been observed in combined EMD and GTR therapy. Using EMD in combination with growth factors, allografts, xenografts and alloplastic material is preferred over using EMD only. The objective of each combination therapy is to increase clinical predictability and improve the outcome of regenerative surgery.
