Abstract
Antikoagulantni su lijekovi kao terapijsko i preventivno sredstvo indicirani u velikom broju cerebrovaskularnih i kardiovaskularnih bolesti. Razlikujemo antikoagulantne i antitrombocitne lijekove. Antikoagulantni lijekovi utječu na fazu koagulacije i kaskadne reakcije, a razlikujemo one koji se primjenjuju oralno (derivati kumarina od kojih je najpoznatiji varfarin i novi antikoagulantni lijekovi dabigatran, rivaroksaban, apiksaban) i one koji se primjenjuju parenteralno (nefrakcionirani heparin, heparin niske molekularne težine, inhibitori trombina). Antitrombocitni lijekovi sprečavaju fazu agregacije trombocita, a najpoznatiji su acetilsalicilna kiselina i klopidogrel. Danas literatura ne preporučuje prekidanje terapije, čak ni dualne terapije za većinu dentalnih zahvata jer se smatra da to povećava rizik od tromboemboličkih komplikacija koji nadmašuje rizik od krvarenja. Postoperativno krvarenje koje se javlja nije fatalno i može se zaustaviti lokalnim mjerama hemostaze.
Abstract (english)
Anticoagulant drugs are prescribed as a therapeutic and preventative agent in a large number of cerebrovascular and cardiovascular diseases. We distinguish anticoagulant and antithrombotic drugs. Anticoagulant drugs affect the coagulation phase and the cascade reaction, and we distinguish those administered orally (coumarin derivatives, of which the most common is warfarin, and new anticoagulant drugs dabigatran, rivaroxaban, and apixaban) from those administered parenterally (unfractionated heparin, low molecular weight heparin, thrombin inhibitors). Antithrombotic drugs prevent the platelet aggregation phase, and the most known are acetylsalicylic acid and clopidogrel. Nowadays, scholarly literature does not recommend discontinuation of therapy, even of a dual one, for most dental procedures, as it is believed that it increases the risk of thromboembolic complications, which exceeds the risk of bleeding. Postoperative bleeding that occurs is not fatal and can be stopped by local hemostasis measures.
