Homologous transplantation or autotransplantation is a surgical procedure that transplants teeth or dental embryos from one alveolar to the other in the same person. The method is most commonly performed in children and adolescents when implant and prosthetic therapy are contraindicated. The third molar, premolar, supernumerary tooth or any tooth which is considered to be of lesser importance is usually chosen as the donor tooth. After the autotransplantation of the dental embryo, it continues to grow and develop a root, creating periodontal and maintaining pulp vitality, and after the transplantation of the mature tooth there is no further growth and root development, and no pulp revascularization is expected. It is of utmost importance to perform a surgical procedure with the maximum atraumatic extraction of the donor teeth and careful manipulation when placing it in the prepared alveolus. The preservation of Hertwig's epithelial root sheath during tooth transplantation is a prerequisite for further growth and root development. After the transplantation, endodontic surgery should be considered, and during each check-up it is necessary to monitor X-rays, vitality, position and mobility of the tooth, the sound of percussion, gingival condition and probing depth. The transplantation is considered unsuccessful if it comes to pulp necrosis, root resorption and tooth ankylosis. The implantation of dental implants demonstrates high degree of success and a predictable long-term result of therapy, but transplantation is a priority among patients with unfinished growth and jaw development. It is a biological procedure in which the teeth, especially during the embryonic stage, have the potential to induce alveolar growth, and can therefore be used with patients before the end of puberty growth, while osseointegrated implants cannot adapt to the patient’s growing jaw.
