Abstract
Pod destruiranim zubima podrazumijevamo zube opsežnog gubitka tvrdog zubnog tkiva i izrazito parodontološki kompromitirane zube. Radi se o zubima upitnih prognoza kojima je alternativa liječenju jedino ekstrakcija, a i kada opstanu, zahtijevaju neki oblik nadogradnje za nadoknadu koronarnog tkiva. Za ekstrakciju i primjenu nadogradnji postoji cijeli niz indikacija i kontraindikacija. Za očuvanje uzimamo u obzir posljedično očuvanje propriocepcije i koštane mase. Parodontološki kompromitirane zube potrebno je parodontološki kompenzirati konzervativnim ili protetskim metodama. Metode pristupa temelje se na povezivanju zuba u blok odnosno splintiranjem. Od mobilnih terapijskih mogućnosti možemo upotrijebiti parcijalnu protezu retiniranu kvačicama ili swing-lock protezu. Fiksnim metodama splintiranja pripadaju krunice povezane u blok i adhezivni mostovi. Ako govorimo o zubima primarno opsežno izgubljenog koronoranog tkiva izrađujemo nadogradnje za kompenzaciju. Možemo upotrijebiti klasične metalne ili nemetalne nadogradnje, Richmond krunice te se možemo poslužiti teleskopskim sustavima. Ako je pak namijenjena pokrovna proteza, za nju izrađujemo itraradikularne pričvrske, osnovne korijenske kapice ili neke od različitih mogućnosti kopči, prečki ili magneta.
Abstract (english)
The term ‘destructed teeth’ implies teeth with severe loss of hard dental tissue and extremely periodontally compromised teeth. The viability of such teeth is questionable, the only alternative to therapy being extraction. Even if the teeth are partially preserved, they require extracoronal restoration. Extraction and posts entail numerous indications and contraindications. When talking about preservation, both preservation of proprioception and preservation of bone mass are taken into consideration. Periodontally compromised teeth need to be periodontally restored in the form of conservative or prosthodontic procedures. Connecting teeth in units, i.e. splinting, presents the basis for methods of approach. Therapeutic options for mobile splinting include partial clasp-retained dentures and swing-lock dentures. Fixed splinting entails crowns connected in units and adhesive bridges. When dealing with teeth with severe hard tissue loss, manufactured restorative posts are used. There are several options such as common metal or non-metal posts, Richmond crowns or telescope systems. If an overdenture is to be made, basic root caps or one of various snap, bar or magnet based attachments are used.
