Abstract
Recesija gingive označava pomicanje mekog tkiva apikalnije od caklinsko-cementnog spojišta, pri čemu površina korijena biva izložena. Osim estetskog, recesije predstavljaju i funkcionalni problem, a liječe se kirurškim postupcima prekrivanja ogoljenog korijena zuba. Postoje različite kirurške metode prekrivanja recesija i različiti alternativni materijali koji se koriste kao zamjena za slobodni vezivni transplantat (SVT). SVT u kombinaciji s koronalno pomaknutim režnjem (KPR) zlatni je standard u terapiji prekrivanja recesija korijena, ali zbog svoje invazivnosti, dodatnog utroška vremena za uzimanje SVT-a s nepca te ograničene količine tkiva koje se može uzeti s nepca, razvijeni su kolageni matriksni materijali. Jedan je od njih i resorbirajući kolageni matrkis (KM), u ovom slučaju OSSIX® VOLUMAX, čiju smo učinkovitost u prekrivanju recesija korijena odlučili usporediti sa SVT-om, pritom isto koristeći KPR. Parametre veličina recesije, dubina sondiranja, širina keratinizirane ginigve i razina kliničkog pričvrstka usporedili smo prije operacije i šest mjeseci nakon operacije. Vrijednosti su izmjerene prvo unutar pojedine grupe, a potom uspoređene međusobno. Dobiveni rezultati pokazuju da je KM + KPR zadovoljavajuća alternativa SVT + KPR kad uzmemo u obzir izostanak morbiditeta koji se postiže uzimanjem transplantata s nepca. U uvjetima u kojima je provedeno istraživanje zaključeno je da SVT + KPR klinički i dalje ima bolje rezultate od KM + KPR, ali neznatno. KM-i mogu biti vrijedna zamjena za SVT.
Abstract (english)
A gingival recession is an apical shift of the marginal soft tissue apical to the cemento-enamel junction leading to root exposure. Apart from an aesthetic point of view, recessions also represent a functional problem and they are treated with surgical procedures aimed at root coverage. There are various surgical procedures for root coverage and different alternative materials which are used as replacements for connective tissue graft (CTG). The CTG with the coronally advanced flap (CAF) is the gold standard in root coverage procedures, but due to its invasiveness and the need to harvest the graft from the palate, which lead to longer surgical time, and limited quantity which can be harvested from the palate, collagen matrix materials have been developed. One of them is the resorbable collagen matrix (CM), in this case, the OSSIX® VOLUMAX, whose efficacy in gingival root recession coverage was compared to the CTG using the CAF approach. We have compared the recession height, the pocket probing depth, the width of the keratinized gingiva, and the clinical attachment level at the base line and after 6 months. The values were measured within each group and then compared. The results showed that the CM+CAF is a satisfying alternative to the CTG+CAF when taking into account the lower morbidity of the patient. In the conditions under which the study was conducted, it was concluded that CTG+CAF clinically still achieves better results than CM+CAF. CM can be a valuable replacement for CTG.
