Abstract
Liječenje rascjepa usne i nepca zahtijeva temeljito razumijevanje različitih dimenzija skrbi kako bi se postigao optimalni ishod kirurškog liječenja. Prema raznim statistikama, učestalost rascjepa usne je oko 1 : 700 novorođene djece. Etiologija rascjepa usne je raznovrsna i uključuje brojne genetske i okolišne čimbenike. Kirurški postupci korekcije rascjepa usne uključuju metode ravne linije, metode geometrijskih linija (trokutaste ili četvrtaste), te metode kliznih i rotacijskih režnjeva, od kojih je najpoznatija Millardova tehnika. Optimalno vrijeme za kiruršku korekciju rascjepa usne je u dobi djeteta od 3 do 6 mjeseci kada je dijete u optimalnoj kondiciji za operacijski zahvat, a također je i dovoljno rano za nesmetani daljnji razvoj struktura lica te funkcija hranjenja i govora. Svrha ovog preglednog rada je prikazati modalitete kirurškog liječenja primarnog rascjepa usne uz naglašavanje kirurških principa i kontroverzi.
Abstract (english)
The treatment of cleft lip and palate requires a thorough understanding of different dimensions of care in order to achieve the optimal outcome of surgical treatment. According to various statistics, the frequency of cleft lip is about 1:700 newborns. The etiology of cleft lip is diverse and includes many genetic and environmental factors. Surgical procedures for the correction of cleft lip include straight line methods, geometrical lines (triangular or square), and sliding and rotational flaps, most notably the Millard's technique. The optimal timing for surgical repair of cleft lip is 3-6 months of age, when the child is in optimal condition for surgery, and is also early enough for undisturbed further development of facial structures as well as feeding and speech functions. The purpose of this review is to show the modalities of surgical treatment of a primary cleft lip with the emphasis on surgical principles and controversies.
