Impacted teeth are those teeth that do not appear inside the dental arch in the expected time estimated for their eruption. This phenomenon is usually asymptomatic, but sometimes it can cause problems such as carious lesions, infections, cysts, etc. The most commonly impacted teeth are the third molars, followed by the maxillary canines, mandibular premolars, and maxillary central incisors. The causes of their impaction are numerous, divided into three basic groups: genetic, systemic and local factors. In order to start impaction therapy as early as possible, early and good diagnosis is very important. Clinical examination, which includes inspection and palpation, reveals a lack of tooth while various radiological methods confirm the existence of impaction. Given the position of a tooth in the jaw, a therapeutic procedure is planned which is extremely complex and requires interdisciplinary cooperation of the oral surgeon, orthodontist and periodontist. Impacted tooth is most often treated with a nonextraction therapeutic procedure that includes orthodontic opening of the space in the dental arch, surgical presentation of the crown and extraction of the tooth in the appropriate position. Three surgical procedures can be used to expose the crown of an impacted tooth: the excision method, the apically displaced lobe method, and the closed eruption method. The choice of a method depends on the type of impacted tooth and its location. A good plan and an individual approach are very important for the beginning of a therapy as it reduces complications such as root resorption, bone loss and poor aesthetics.
