Abstract
Sinuitisi su upale paranazalnih sinusa izazvane infekcijama (virusnim, bakterijskim ili gljivičnim) ili alergijskim reakcijama. Najčešće su rinogene etiologije, no mogu biti i odontogeni. Odontogeni se razvijaju kao posljedica povrede Schneiderove membrane, mukoperiosta maksilarnog sinusa, iz smjera dentoalveolarnog nastavka. Prepoznavanje uzroka od iznimne je važnosti jer su odontogeni sinuitisi jedinstveni po patofiziologiji, mikrobiologiji, dijagnostici i terapiji. Za pravilnu obradu pacijenta potreban je interdisciplinarni pristup – suradnja otorinolaringologa, doktora dentalne medicine i radiologa. Dijagnostika podrazumijeva detaljnu medicinsku i stomatološku anamnezu, klinički pregled, stomatološki status te radiološko snimanje. Terapija odontogenih sinuitisa sastoji se od antibiotika, kirurškoga zahvata na sinusu te stomatološkoga zahvata kojim se eliminira sam izvor bolesti. Neprepoznati uzrok onemogućava cjelovitost terapije i bolest perzistira te postoji veća opasnost od razvoja komplikacija.
Abstract (english)
Sinusitis is inflammation of the sinuses, caused by an infection (viral, bacterial or fungal) or by an allergy. More often they are rhinogenic but can also be odontogenic. Odontogenic sinusitis is the result of disruption of the Schneiderian membrane, mucoperiosteum of the maxillary sinus, caused by dental pathology or dental procedures. Recognition of the underlying cause is of utmost importance because odontogenic sinusitis is different from rhinogenic and requires special treatment. Interdisciplinary approach is needed. The cooperation of an otorhinolaryngologist, dentist and radiologist is best. Medical history, both general and dental, clinical examination, dental status and radiological imaging allow proper diagnosis. Therapy of odontogenic sinusitis consists of antibiotics, sinus surgery and dental procedures which directly eliminate the cause of the inflammation. If the cause remains unrecognised, therapy will be unsuccessful, and the inflammation persists, likely developing complications.
