The mucogingival problem is denoted by a combination of three characteristics: the presence of inflammation, gingival recession, and lack of keratinized gingiva. Gingival recession is defined as the displacement of soft tissue apical to the enamel-cement junction with the exposure of the root surface. Because smile aesthetics are becoming increasingly important to the patients there are more and more indications for mucogingival surgery in recent years. In addition to the aesthetics of a smile, the reasons for repairing gingival recessions are hypersensitivity, root caries and tooth abrasion. The main goals of mucogingival surgery are to stop the recession and cover it, widen the band of attached gingiva and deepen the vestibule. Covering of the recession is one of the most common interventions nowadays, primarily due to aesthetic indications. Apart from being an aestethic problem, gingival recessions also represent a functional problem, and are etiologically associated with various genetic and acquired factors. Treatment of gingival reccesions includes explaning the etiology of the problem, a change in oral hygiene techniques, and mucogingival surgical procedures. Mucogingival surgical procedures are applied only if there is no improvement after a change in the oral hygiene technique over several months, with regular patient check-ups. The goal of surgical therapy is to cover the exposed root of the tooth and regenerate all lost periodontal tissues, using the techniques of pedicle and free soft tissue transplantation. Free transplant techniques include free gingival graft and free connective graft. Due to the predictable results, improved aesthetics and less invasive wound on the palate, free connective graft has become today's gold standard in covering gingival recessions.
