Abstract
Žlijezde slinovnice su egzokrine žlijezde koje se nalaze u usnoj šupljini i luče slinu koja ima probavnu, mehaničku, okusnu, govornu i antimikrobnu ulogu. Općenito ih se dijeli prema veličini na velike i male žlijezde slinovnice. Maligni tumori žlijezda slinovnica predstavljaju oko 5 % svih tumora glave i vrata i često su lošeg ishoda pa je pri dijagnosticiranju i određivanju plana terapije potrebno opsežno znanje. Maligni tumori s najvišom incidencijom su mukoepidermoidni karcinom, adenoid cistični karcinom i adenokarcinom. Za dijagnostiku površinski lokaliziranih lezija koristi se ultrazvuk, dok se za lezije nedostupne ultrazvuku i točniju lokalizaciju te određivanje odnosa tumora s okolnim anatomskim strukturama koristi magnetna rezonancija. Terapijske metode uključuju kiruršku resekciju malignog tumora, isto kao i radioterapiju za pacijente s opsežnijim promjenama. Također se koristi i sistemna terapija, u vidu kemoterapije i molekularne ciljane terapije koja je još u razvoju.
Abstract (english)
Salivary glands are exocrine glands located in the oral cavity. Their role is to produce saliva, which has antimicrobial, digestive, mechanic, taste and speaking function. There are two types of salivary glands according to their size - major and minor. Malignant tumors of salivary glands are very rare, in particular they represent about 5 % of all head and neck tumors. Knowledge and experience in diagnosing and planning the therapy are extremely important as their outcome can often be irreparable. The most common malignant tumors of salivary glands are mucoepidermoid carcinoma, adenoid cystic carcinoma and adenocarcinoma. The ultrasound is used for diagnosing superficial lesions, while magnetic resonance is used for lesions unapproachable by ultrasound, as well as for more precise localization and for determining the tumor's relations to the surrounding anatomical structures. Therapy includes surgical resection of the malignant tumor and radiotherapy for patients with extensive lesions, as well as systemic therapy, such as chemotherapy and targeted molecular therapy, which is still under development.
