Abstract (english)
Background Professional practice is commonly systematised in practice guidelines and in text books. However, much practice knowledge is held at a tacit level and rarely documented in the research literature. It is therefore challenging to compare or to fully evaluate practice decisions. The aim of the study was to explore therapists’ understanding and experiences of common decisions. This poster focuses on discharge decisions. Methods The study used a qualitative, phenomenological design.... More Firstly, 10 therapists working in Suvag Polyclinic in Zagreb were interviewed using semi-structured interviews. Secondly, 11 therapists from across Croatia, attended a focus group. Participants were all qualified therapists with experience of working with children with Developmental Language Disorder. Interview participants were asked to describe their discharge decision in a recent case. Probe questions elicited further details and examples. Interviews were transcribed (three were translated into English). Questions to the focus group aimed to validate the preliminary findings and to pursue explanations and rationales. The analysis identified therapists’ definitions and understanding of discharge, the factors influencing their decision and theories of practice that shaped their discharge actions. Results Although discharge is mostly seen as a termination of therapy, understanding of discharge slightly varied across the sample. Therapists emphasized that discharge of children with DLD is mostly what Hersch (2010) phrased as ‘unideal’ where plateauing, child’s achievements, motivation for the therapy and support were seen as decisive factors leading to discharge decision, together with the length of the therapy, frequency and continuity of the therapy. Factor’s interactions and hierarchies shaped different patterns of clinician’s practice. Conclusions The findings provide a preliminary understanding of the tacit knowledge therapists use when discharging children from therapy. Further studies, using similar methodology across different countries and settings could give us full understanding of the decision-making process in discharge from SLT and support planning an optimal service delivery.
