Abstract Šećerna bolest jedan je od najvećih javnozdravstvenih problema današnjice. Prema najnovijim podacima broj oboljelih u svijetu iznosi oko 425 milijuna dok je u Hrvatskoj registrirano oko 284.000 bolesnika. Šećernu bolest odlikuje povećana razina glukozu u krvi, koja nastaje kao posljedica manjka lučenja inzulina iz gušterače ili njegovog nedostatnog učinka, odnosno inzulinske rezistencije. Dvije glavne skupine šećerne bolesti su šećerna bolest tip 1 i tip 2. Zbog kroničnog i progresivnog tijeka, praćenog pojavom kroničnih komplikacija, šećerna bolest u velikoj mjeri određuje način života bolesnika i utječe na kvalitetu života. Osim na tjelesno, šećerna bolest utječe i na psihičko zdravlje te izaziva pojavu psihopatoloških simptoma. Ovo istraživanje poduzeto je u svrhu je ispitivanja međuodnosa psihopatoloških simptoma i kvalitete života bolesnika sa šećernom bolesti na uzorku sudionika iz Sveučilišne klinike Vuk Vrhovac. U istraživanju je sudjelovalo 98 sudionika: 51 pacijent sa šećernom bolesti tip 1 i 47 pacijenata sa šećernom bolesti tip 2. Kao mjerni instrumenti korišteni su Upitnik socio demografskih i medicinskih obilježja, Skraćena verzija Upitnika kvalitete života Svjetske zdravstvene organizacije i Crown-Crispov indeks iskustva. Rezultati su pokazali da oboljeli od tipa 1 iskazuju veće zadovoljstvo tjelesnim zdravljem dok oboljeli od tipa 2 iskazuju više simptoma somatskih manifestacija anksioznosti. Utvrđeno je da Zadovoljstvo zdravljem, Tjelesno zdravlje i Socijalni odnosi utječu na vrijednost HbA1c pri čemu je viša vrijednosti HbA1c povezana s manjim zadovoljstvom na domeni Zadovoljstva zdravljem i Tjelesnim zdravljem te sa većim zadovoljstvom na domeni Socijalnih odnosa. Varijable Dob, Tjelesno zdravlje i Somatske manifestacije anksioznosti predstavljaju značajne diskriminacijske čimbenike pri čemu su oboljeli od šećerne bolesti tip 2 stariji te iskazuju više simptoma somatske manifestacije anksioznosti, dok veće zadovoljstvo tjelesnim zdravljem iskazuju oboljeli od šećerne bolesti tip 1. Naposljetku, logistička regresijska analiza pokazala je da povećanje dobi povećava šansu za šećernu bolest tip 2, kao i povećanje varijable anksioznost u fobijama.
Abstract (english) Diabetes is a group of diseases caused by defects in insulin secretion and/or action, characterized by chronic hyperglycemia and associated with the development of chronic complications. As the prevalence has reached epidemic proportion, with 425 millions of patients worldwide, diabetes is considered as one of the major health issues. In Croatia 285 thousands adults are living with Diabetes. Two major forms are type 1 and type 2 Diabetes. Due to the chronic and progressive nature, diabetes is closely associated with patients lifestyle, largely determining their quality of life. In addition to physical, diabetes affects psychological health leading to the appearance of psychopathological symptoms. The aim of this research was to investigate the interdependence of psychopathological symptoms and the quality of life between diabetic patients from the the University Clinic Vuk Vrhovac. Total of 98 patients were included: 51 with type 1 and 47 with type 1 diabetes. The instruments used consisted of The socio-demographic and medical characteristics questionnaire, shortened version of the World Health Organization Quality of Life Instrument and Crown- Crisp Experiential Index. The Results indicate that type 1 patients showed greater satisfaction with Physical Health while type 2 patients exhibited more symptoms of Somatic Anxiety. It was found that Health Satisfaction, Physical Health and Social Relationship affected the value of HbA1c, with higher HbA1c being associated with less satisfaction in the Health Satisfaction and Physical Health while greater satisfaction in the field of Social Relationship. Age, Physical Health and Somatic Anxiety appeared as significant discriminatory factors in which type 2 diabetic patients have shown more symptoms of Somatic Anxiety, while greater satisfaction with Physical Health has been indicated by type 1 diabetes. Finally, logistic regression analysis showed that increase in age was associated with the likelihood of type 2 diabetes, and the increase in the variable of Phobic Anxiety.
