Abstract Uvod
Glaukom je kronična progresivna optička neuropatija s promjenama na vidnom živcu i progresivnim gubitkom vidnog polja i vida. Povezana je s povišenim očnim tlakom i uzrokuje 12% svih sljepoća u svijetu. Ne može se izliječiti, ali uz odgovarajuću terapiju i redovite kontrole oboljelih, tijek bolesti može se dobro kontrolirati.
Cilj
Na osnovu prikupljenih demografskih podataka ispitanika, podataka o suradljivosti u liječenju (redovitost uzimanja antiglaukomske terapije i redovito dolaženje na kontrolne preglede), podataka o kliničkim ishodima bolesti (vidna oštrina, ispitivanje vidnog polja, visina intraokularnog tlaka), te prisutnosti straha od gubitka vida kod ispitanika, cilj istraživanja bio je:
- utvrditi postoji li razlika u kliničkim ishodima bolesti i prisutnosti straha od gubitka vida, između osoba oboljelih od glaukoma koje surađuju u liječenju i onih koje ne surađuju,
- analizirati povezanost suradljivosti s demografskim značajkama ispitanika.
Metode
Istraživanjem je obuhvaćeno 100 ispitanika (200 očiju) oboljelih od glaukoma, a provedeno je na Zavodu za očne bolesti KB "Dubrava" u Zagrebu, Hrvatska. Demografski podatci (dob, spol, stupanj obrazovanja, stambeni status), podatci o suradljivosti u liječenju (redoviti dolasci na kontrolne preglede i redovitost uzimanja antiglaukomske terapije), te podatci o strahu od gubitka vida prikupljeni su putem intervjua s ispitanicima „licem u lice“. Podatci vezani uz klinički ishod (vidna oštrina, indeksi ispitivanja vidnoga polja, vrijednosti intraokularnog tlaka) preuzeti su iz oftalmoloških nalaza svakoga ispitanika.
Rezultati
Rezultati istraživanja pokazuju da je uočena statistički značajna razlika s obzirom na redovito uzimanje antiglaukomske terapije i sociodemografske pokazatelje (p<0,05). Također je uočena statistički značajna razlika za indekse ispitivanja vidnog polja s obzirom na strah od gubitka vida, pri čemu je 26% ispitanika imalo prisutan jako izražen strah od gubitka vida (6-10 na skali od 1-10). Nije utvrđena statistički značajna razlika kod promatranih pokazatelja (vidne oštrine, intraokularnog tlaka i indeksa ispitivanja vidnog polja) s obzirom na spol i stupanj obrazovanja ispitanika.
Većina ispitanika, bez obzira na dob, spol i stupanj obrazovanja, dobro surađuje u liječenju glaukoma, najviše zbog straha od gubitka vidnog polja i nastanka sljepoće.
Abstract (english) Title
Correlation between treatment compliance and clinical outcomes in patients with glaucoma.
Background
Glaucoma is a chronic progressive optic neuropathy with changes in the optic nerve and progressive loss of visual field and vision. It is associated with high intraocular pressure and causes 12% of all blindness in the world. It cannot be cured, but with proper therapy and regular monitoring of patients, the course oft he disease can be well controlled.
Aim
Based on the collected demographic data of the patient, data on cooperation in treatment (regularity of taking antiglaucoma therapy and regular attendance at check-ups), data on clinical outcomes of the disease (visual acuity, visual field examination, intraocular pressure) and the presence of fear of vision loss in the patients , the aim of the research is:
- to determine whether there is a difference in the clinical outcomes of the disease and the presence of fear of vision loss, between cooperating and non-cooperating glaucoma patients,
- to analyze the connection of cooperation with the demographic characteristics of the patients.
Methods
The study included 100 patients (200 eyes) with glaucoma, and was conducted at the Department of Ophthalmology University Hospital „Dubrava“, in Zagreb, Croatia. Demographic data (age, gender, education level, housing status), data on treatment collaboration (regular visits to check-ups and regularity of taking antiglaucoma therapies) and the presence of fear of vision loss were collected through „face-to-face“ interviews. Data on clinical outcomes (visual acuity, visual field examination indices, intraocular pressure values) were collected from ophthalmological examinations of each patient.
Results
The results of the research show that a statistically significant difference was observed with regard to regular use of antiglaucoma therapy and sociodemographic indicators (p<0.05). There was also a statistically significant difference for visual field examination indices with respect to fear of vision loss, with 26% of subjects having a very pronounced fear of vision loss (6-10 on a scale of 1-10). No statistically significant difference was found in the observed indicators (visual acuity, intraocular pressure and visual field examination index) with regard to the gender and level of education of the patients.
Conclusion
According of the results of the research, we can see that statistically very few patients do not cooperate in the treatment of glaucoma, and that a statistically significant majority of patients, regardless of age, gender and level of education, cooperate well in treatment, mostly for fear of loss of visual field and the development of blindness.
