Abstract
Facijalni živac sedmi je moždani živac koji izlazi između ponsa i produžene moždine u pontocerebelarnom kutu. Charles Bell je utvrdio da je periferna facijalna paraliza posljedica zahvaćenosti sedmog kranijalnog živca. Bellova paraliza rezultira djelomičnom ili potpunom nemogućnošću pokretanja mimčnih mišića polovice lica. Iako se obično povlači u roku od nekoliko tjedana ili mjeseci, Bellova paraliza može dovesti do ozbiljne oralne insuficijencije i nemogućnosti zatvaranja kapaka u nekim slučajevima, što može dovesti do potencijalno trajne ozljede oka. Pridruženi simptomi uključuju hiperakuziju, suho oko, stražnju aurikularnu bol, promijenjen okus i smanjeno suzenje. Liječenje Bellove paralize uključuje korištenje kortikosteroida i antivirusnih lijekova. Dodatno liječenje uključuje tretiranje oftalmoloških i oralnih simptoma, fizioterapiju, razmatranje kirurškog zahvata i liječenje posljedica kao što su sinkinezija lica i depresija. Paraliza facijalnog živca može se poboljšati do godinu dana kasnije. Bolesnici s nepotpunom paralizom imaju bolju prognozu od pacijenata s potpunom paralizom i što je pacijent mlađi to je prognoza bolja. U bolesnika s nepotpunom paralizom do 94% se potpuno oporavi. Za starije bolesnike i one sa teškom slabošću ishod je nepovoljniji.
Abstract (english)
The facial nerve is the seventh cranial nerve that exits between the pons and medulla oblongata in the pontocerebellar angle. Sir Charles Bell established that peripheral facial palsy is due to involvement of the seventh cranial nerve. Bell's palsy results in a partial or complete inability to move the facial muscles of half of the face. Although it usually resolves within a few weeks or months, Bell's palsy can lead to severe oral insufficiency and the inability to close the eyelids in some cases, leading to potentially permanent eye damage. Associated symptoms include hyperacusis, dry eye, posterior auricular pain, altered taste, and decreased lacrimation. Treatment of Bell's palsy includes the use of corticosteroids and antiviral drugs. Additional treatment includes treatment of ophthalmic and oral symptoms, physiotherapy, consideration of surgery, and treatment of sequelae such as facial synkinesia and depression. Facial nerve palsy may improve up to a year later. Patients with partial paralysis have a better prognosis than patients with complete paralysis and the younger the patient, the better the prognosis. In patients with incomplete paralysis, up to 94% recover completely. For older patients and those with severe weakness, the outcome is less favorable.
