Abstract Zbog svoje velike pokretljivosti, rameni zglob pripada među najnestabilnije zglobove ljudskog tijela. Stabilizaciju ramena osigurava tetivno – mišićna ovojnica naziva rotatorna manšeta. Rotatornu manšetu čine mišić supraspinatus, infraspinatus, teres minor te subskapularis. Osobe koje mnogo koriste ruku, posebno u položaju iznad glave, imaju veću vjerojatnost od nastanka oštećenja. Sindrom sraza ramena predstavlja stanje u kojem dolazi do sužavanja subakromijalnog prostora zbog čega se javljaju oštećenja na tetivama i drugim strukturama koje se nalaze u suženom subakromijalnom prostoru. Tetive mišića supraspinatusa i infraspinatusa spadaju među strukture koje su najčešće zahvaćene tijekom sindroma sraza u ramenu. Ponavljanim pokretima iznad glave, posebno abdukcije i elevacije stvara se preduvjet za sužavanje prostora između akromiona i velike kvrge zbog čega može doći do patološkog stanja u području ramenog zgloba. Sindrom sraza u ramenu karakterizira bol i smanjen opseg pokreta. Najintenzivniji bolovi javljaju se prilikom pokreta abdukcije između 70° i 120°. Ispružen položaj ruke i spavanje na zahvaćenoj strani pogoršava stanje, a kao posljedica se može javiti slabost i ukočenost segmenta. Kod procjene sindroma sraza kao primarna metoda smatraju se specifični testovi kojima će se sa sigurnošću utvrditi vrsta i karakteristike samog oštećenja. Liječenje sindroma sraza u ranoj fazi temelji se na analgetskim metodama rehabilitacije, a kasnije kao primarna metoda smatra se terapijsko vježbanje, odnosno vježbe jačanja i istezanja kojima će se vratiti stabilnost i mobilnost zglobu te omogućiti osobi vraćanje normalnih funkcionalnih sposobnosti.
Abstract (english) Due to its great mobility, the shoulder joint is among the most unstable joints of the human body. Stabilization of the shoulder is provided by a muscle-tendon structure called the rotator cuff. The rotator cuff consist of the supraspinatus, infraspinatus, teres minor and subskapularis muscles. People who use their hands a lot, especially in the overhead position, are more likely to develop damage. Shoulder impingement syndrome is a condition in which there is a narrowing of the subacromical space, which causes damage to the tendons and other strucutres located in the narrowed subacromical space. The tendons of the supraspinatus and infraspinatus muscles are among the structures that are most often affected during shoulder impingement syndrome. Repeated movements above the head, especially abduction and elevation, create a prerequisite for the narrowing of the space between the acromion and the big tubercle, which can lead to a pathological condition in the area of the shoulder joint. Shoulder impingement syndrome is characterized by pain and reduced range of motion. The most intense pain occurs during the abduction movement between 70° and 120°. The outstretched position of the arm and sleeping on the affected side worsens the condition, and as a result, weakness and stiffness of the segment may occur. Specific tests are considered as the primary method for the assessment of impingement syndrome, which will determine with certainty the type and characteristics of the damage itself. The treatment of impingement syndrome in the early stages is based on analgesic methods of rehabilitation, and later the primary method is considered to be therapeutic exercises, or in other words strengthening and stretching exercises that will restore stability and mobility to the joint and allow the person to return to normal functional abilities.
