Abstract Prema definiciji koju je predložila Svjetska zdravstvena organizacija moždani udar je brzo razvijanje kliničkih znakova žarišnog (ili globalnog) poremećaja cerebralne funkcije, sa simptomima koji traju 24 sata ili dulje, ili dovode do smrti, bez vidljivog uzroka osim vaskularnog porijekla. Moždani udar je drugi vodeći uzrok smrti u svijetu i treći vodeći uzrok i invaliditeta. Dob je najznačajniji nepromjenjivi čimbenik rizika za nastanak moždanog udara. S produljenjem očekivanog životnog vijeka, očekuje se da će većina bolesnika s moždanim udarom biti starije ili čak vrlo starije osobe. Zbog visoke smrtnosti i morbiditeta kod starijih pacijenata, moždani udar je danas svjetski zdravstveni problem koji iziskuje ogromne financijske izdatke, jer više od polovice preživjelih ima teški i trajni invaliditet. Vjeruje se da su medicinske komplikacije važan problem nakon akutnog moždanog udara i predstavljaju potencijalne prepreke optimalnom oporavku. Unatoč napretku u modernoj medicini, lijekovima i medicinskoj tehnologiji, bolesti moždanog udara predstavljaju znatan rizik od mortaliteta i morbiditeta za pojedinca s povećanim ekonomskim opterećenjem za društvo. Zbog toga postoji hitna potreba za razvojem optimalnog plana upravljanja bolešću moždanog udara, koji uključuje sveobuhvatan program rehabilitacije nakon moždanog udara. Promijenjeni načini skrbi i povećana uključenost pacijenata u samozbrinjavanje nakon bolesti rezultirali su promjenama u specifičnim ulogama i funkcijama medicinskih sestara/tehničara u rehabilitaciji. Iako je sestrinsku profesiju karakterizirala sposobnost postizanja, održavanja ili oporavka optimalnog zdravlja i kvalitete života, opis uloga i funkcija sestrinstva u rehabilitaciji nije čest. Napredne i specijalizirane uloge medicinskih sestara/tehničara za moždani udar relativno su novi fenomen i prepoznate su kao značajan doprinos medicinskoj sestrinskoj procjeni u hitnim službama, provođenju trombolize, provođenju intervencija kod urinarne inkontinencije, disfagije, prehrane i hidracije te pozicioniranja i mobilizacije.
Abstract (english) According to the definition proposed by the World Health Organization, a stroke is the rapid development of clinical signs of a focal (or global) disorder of cerebral function, with symptoms lasting 24 hours or longer, or leading to death, without an apparent cause other than vascular origin. Stroke is the second leading cause of death in the world and the third leading cause of disability. Age is the most significant non-modifiable risk factor for stroke. With increasing life expectancy, the majority of stroke patients are expected to be elderly or even very elderly. Due to the high mortality and morbidity in elderly patients, stroke is today a worldwide health problem requiring huge financial expenditures, as more than half of the survivors have severe and permanent disability. Medical complications are believed to be an important problem after acute stroke and represent potential barriers to optimal recovery. Despite advances in modern medicine, drugs, and medical technology, stroke diseases pose a significant risk of mortality and morbidity to the individual with an increased economic burden on society. Therefore, there is an urgent need to develop an optimal stroke disease management plan, which includes a comprehensive rehabilitation program after stroke. Changed care methods and increased involvement of patients in self-care after the disease have resulted in changes in the specific roles and functions of nurses/technicians in rehabilitation. Although the nursing profession was characterized by the ability to achieve, maintain or recover optimal health and quality of life, the description of the roles and functions of nursing in rehabilitation is not common. The advanced and specialized roles of stroke nurses/technicians are a relatively new phenomenon and are recognized as a significant contribution to nursing assessment in the emergency department, administration of thrombolysis, administration of interventions for urinary incontinence, dysphagia, nutrition and hydration, and positioning and mobilization.
