Abstract Fizioterapija igra značajnu ulogu u rehabilitaciji žena nakon ginekoloških zahvata, kao i u
liječenju drugih ginekoloških stanja. Fizioterapeuti rade na obnavljanju funkcije mišića dna
zdjelice, trbušnih mišića i mišića kralježnice kroz individualno prilago đen terapijski plan i
program. Edukacija pacijentica također je važan dio procesa kako bi se pacijentice osnažile i
naučile kako pravilno koristiti mišiće zdjelice u svakodnevnim aktivnostima. Mišići zdjeličnog
dna odgovorni su za podršku zdjeličnim organima kroz koordinirane kontrakcije i relaksacije.
Pružaju aktivnu podršku kroz stalno stanje mišićne kontrakcije i pasivnu podršku okolnog
vezivnog tkiva i fascije. Mišići zdjeličnog dna refleksno se stežu prema gore i zatvaraju vaginu,
mokraćni mjehur i analni sfinkter kod povećanja intraabdominalnog tlaka, što je važno za
održavanje kontinencije. Strategija liječenja mora biti povezana s ciljevima klijenta i usredotočiti
se na ono što joj je važno. To može pomoći u poticanju općeg angažmana. Također je važno
potaknuti klijenta da se osjeća povezano sa svojim tijelom i da osjeća kako ima moć promijeniti
svoj sustav. Početni tretman započinje svjesnošću i povećanjem somatske svjesnosti. Ova
strategija ponovnog povezivanja uma i tijela je značajna jer žene s poremećajima zdjeličnog dna
mogu imati neku "razdvojenost" prema svojem zdjeličnom dnu. Buđenje / ponovno povezivanje
zdjeličnog dna, stoga, predstavlja neophodnu početnu točku. Važno je istražiti kako emocionalno
zdravlje, mentalno zdravlje, odnosi i duhovno zdravlje utječu na tjelesno zdravlje u ovih
pacijentica. Pored toga, važno je uzeti u obzir kako čimbenici poput sna, prehrane, hidracije i
kretanja međusobno djeluju pri radu na obnavljanju zdravlja zdjelice.
Abstract (english) Physiotherapy plays a significant role in the rehabilitation of women following gynecological
procedures, as well as in the treatment of other gynecological conditions. Physiotherapists work
on restoring the function of the pelvic floor muscles, abdominal muscles, and spinal muscles
through individually tailored therapeutic plans and programs. Patient education is also an
important part of the process to empower patients and teach them how to properly engage their
pelvic muscles in daily activities. The pelvic floor muscles are responsible for supporting the pelvic
organs through coordinated contractions and relaxations. They provide active support through a
constant state of muscle contraction and passive support through the surrounding connective tissue
and fascia. The pelvic floor muscles reflexively tighten upwards and close the vagina, urinary
bladder, and anal sphincter when intra-abdominal pressure increases, which is important for
maintaining continence. The treatment strategy should be related to the client's goals and focus on
what is important to her. This can help encourage overall engagement. It is also important to
encourage the client to feel connected to her body and feel that she has the power to change her
system. The initial treatment begins with awareness and increasing somatic awareness. This mindbody
reconnection strategy is significant because women with pelvic floor disorders may have
some "disconnect" to their pelvic floor. Awakening / reconnecting the pelvic floor is therefore a
necessary starting point. It is important to explore how emotional health, mental health,
relationships, and spiritual health influence physical health in these patients. Additionally,
considering factors such as sleep, nutrition, hydration, and movement and how they interact when
working to restore pelvic health is crucial.
