Abstract Disfagija je česta posljedica moždanog udara. U različitim istraživanjima podaci se kreću između 8 % i 81 %. Povećava rizik od nastanka komplikacija kao što su dehidracija, malnutricija, aspiracija, aspiracijska pneumonija pa i smrt. Rano otkrivanje i prilagođavanje liječenja poremećaja gutanja smanjuje rizik od nastanka komplikacija, a utječe i na duljinu hospitalizacije bolesnika. Cilj rada je utvrditi učestalost disfagije bolesnika na Odjelu neurologije Opće bolnice Pula, te istražiti fenomenologiju poremećaja gutanja kod bolesnika s dijagnozama ishemijskog i hemoragijskog moždanog udara te razliku u rezultatima između muških i ženskih bolesnika. Istraživanje je provedeno pomoću EAT-10 skale za procijenu gutanja. Sudjelovalo je 99 bolesnika koji su doživjeli moždani udar i boravili na Odjelu neurologije u razdoblju od svibnja do srpnja 2022. godine. Bolesnicima je obavljena procjena prilikom prijema i otpusta kako bi se usporedbom rezultata dobio podatak o oporavku bolesnika. Za obradu podataka korišten je statistički program IBM SPSS Statistics 27. Korištene su metode deskriptivne statistike (frekvencije odgovora, aritmetičke sredine/prosječne vrijednosti i standardne devijacije, minimum i maksimum), dok je za provjeru normalnosti distribucije rezultata korišten Kolomogorov-Smirnov test.
Rezultati: prilikom prijema i otpusta bolesnika s obje vrste moždanog udara, učestalost poremećaja gutanja veća je od 40 %. Muški su bolesnici već prilikom prijema izražavali manje problema povezanih s gutanjem, dok žene izražavaju ozbiljnije probleme, u nekim je područjima došlo i do blažeg porasta ozbiljnosti. Muški bolesnici u odnosu na bolesnice imaju bolje rezultate. S obzirom na mali udio bolesnika s dijagnozom hemoragijskog moždanog udara, sve statističke obrade podataka provedene na navedenom uzorku potrebno je interpretirati s posebnim oprezom.
Zaključak je da kod bolesnika s moždanim udarom postoji povećan rizik za disfagiju i samim tim imogućnost razvoja komplikacija i produljenog i otežanog liječenja. Rana dijagnostika i prilagodba intervencija kao i multidisciplinarni pristup bitno određuju ishod liječenja i oporavka bolesnika.
Abstract (english) Dysphagia is a common side effect of stroke. In different studies, the data ranges between 8% and 81%. Dysphagia increases the risk of complications such as dehydration, malnutrition, aspiration, aspiration pneumonia and even death. Early detection and adjustment of treatment for swallowing disorders reduces the risk of complications and also affects the length of the patient's hospitalization. The aim of this work is to determine the frequency of dysphagia in patients at the Neurology Department of Pula General Hospital and to investigate the phenomenology of swallowing disorders in patients diagnosed with ischemic and hemorrhagic stroke and the difference in results between male and female patients. Methods: The research was conducted using the EAT-10 scale for the assessment of swallowing. 99 patients who experienced a stroke and stayed at the Neurology department from May to July 2022 participated. The patients were assessed during admission and discharge in order to obtain information about their recovery by comparing the results. The statistical program IBM SPSS Statistics 27 was used for data processing. Descriptive statistics methods were used (response frequencies, arithmetic means/average values and standard deviations, minimum and maximum), while the Kolomogorov-Smirnov test was used to check the distribution of the results. Results: during the admission and discharge of patients with both types of stroke, the frequency of swallowing disorders is greater than 40%. Male patients already expressed fewer problems related to swallowing at the time of admission, while women expressed more serious problems, in some areas, there was also a slight increase in severity. Male patients have better results than female patients. Considering the small proportion of patients with a diagnosis of hemorrhagic stroke, all statistical data processing performed on the mentioned sample should be interpreted with caution. Conclusion is that in patients with a stroke, there is an increased risk for dysphagia and, therefore, the possibility of developing complications and prolonged and difficult treatment. Early diagnosis and adjustment of interventions as well as a multidisciplinary approach significantly determine the outcome of treatment and recovery of the patient.
