Abstract Unatoč velikom napretku u području medicine i posljedičnom sniženju mortaliteta, djecu ne možemo uvijek izliječiti. U takvim okolnostima primjenjuje se palijativna skrb. Palijativna skrb djece usko je povezana s palijativnom skrbi za odrasle i uključuje sveobuhvatnu i potpunu brigu o tijelu, umu i duhu neizlječivo bolesnog djeteta. Prvenstveno je usmjerena na postizanje optimalne kvalitete života oboljelog djeteta. Trebala bi započeti odmah po postavljanju dijagnoze neizlječive bolesti, a ne tek kada bolest uđe u terminalnu fazu. Uvijek uključuje i roditelje koji su značajan izvor informacija o simptomima bolesti, zdravstvenom stanju, osjećajima djeteta, ali i teškoćama s kojima se suočavaju. Specifičnost pedijatrijske palijativne skrbi je i u tome što su angažman roditelja, emocionalne reakcije djece i roditelja, ali i potreba za podrškom drugih, više naglašeni nego u odrasle populacije. Neizlječiva bolest i blizina smrti djeteta uvijek su na svojevrstan način teško prihvatljivi za obitelj, ali i za zdravstvene djelatnike. Razdoblje trajanja bolesti i približavanja kraju života često je vrlo dugačko i neizmjerno stresno. Zato je iznimno važno uz ublažavanje tjelesnih simptoma pružiti i podršku u ublažavanju psihosocijalnih teškoća koje često nisu na vrijeme prepoznate. U nastojanju postizanja optimalne kvalitete života te ostvarenju drugih ciljeva palijativne skrbi jedna od važnijih komponenti je i kvalitetna komunikacija između oboljelog djeteta, zdravstvenog osoblja i roditelja. Pri tome medicinska sestra ima krucijalnu ulogu jer upravo ona provodi najviše vremena s bolesnim djetetom i njegovom obitelji. Kao dio multidisciplinarnog tima aktivno sudjeluje u planiranju, koordiniranju, provođenju i evaluaciji pedijatrijske palijativne skrbi pružajući tako bezuvjetnu podršku na svim razinama zdravstvene zaštite.
Abstract (english) Despite great advances in medicine and the consequent reduction in mortality, children cannot always be cured. In such circumstances, palliative care is applied. Palliative care for children is closely related to palliative care for adults and includes comprehensive and complete care for the body, mind and spirit of a terminally ill child. It is primarily aimed at achieving the optimal quality of life of a sick child. It should be started immediately after the diagnosis of an incurable disease, and not just when the disease enters the terminal phase. It always includes parents who are a significant source of information about the symptoms of the disease, health condition, feelings of the child, but also the difficulties they face. The specificity of pediatric palliative care is that the involvement of parents, emotional reactions of children and parents, but also the need for support from others, are more pronounced than in the adult population. An incurable disease and the imminent death of a child are always difficult to accept for the family, but also for health professionals. The period of disease duration and approaching the end of life is often very long and immensely stressful. That is why, in addition to alleviating physical symptoms, it is extremely important to provide support in alleviating psychosocial difficulties that are often not recognized in time. In an effort to achieve an optimal quality of life and achieve other goals of palliative care, one of the most important components is quality communication between the sick child, health care staff and parents. The nurse has a crucial role in this because she spends the most time with the sick child and his family. As part of a multidisciplinary team, she actively participates in the planning, coordination, implementation and evaluation of pediatric palliative care providing unconditional support at all levels of health care.
