Abstract Psorijatični artritis je kronična upalna artropatija koja može zahvatiti aksijalni skelet i periferne zglobove. Psorijatični artritis karakteriziraju monoartritis, asimetrični oligoartritis, nepravilna devijacija prstiju, kobasičasta oteklina, odsutnost reumatoidnog faktora i supkutanih čvorića, često resorptivne i destruktivne skelete promjene šake i stopala, spondilitis, afekcija sakroilijakalnih zglobova te psorijatične promjene na vlasištu, koži i noktima. Psorijaza se javlja znatno češće u osoba s artitisom nego u populaciji. Dokazano je da je prevalencija psorijaze 2-3% u populaciji. Visoka obiteljska agregacija bolesti očito upućuje na važnost genetskog faktora, i to više njih. Međutim valja kazati da sve osobe, nosioci genetičke predispozicije, ne obolijevaju uvijek od psorijatičnog artritisa. Bolest se javlja najčešće između 20. i 40. godine života. Fizioterapijska procjena kod psorijatičnog artritisa sastoji se od subjektivnog pregleda, objektivnog pregleda, postupaka mjerenja i testova, te plana terapije. Liječenje psorijatičnog artritisa uvijek je kombinirano, a sastoji se od općih mjera, općeg i lokalnog medikamentoznog liječenja, fizikalne terapije i, po potrebi, kirurškog liječenja. Cilj liječenja psorijatičnog artritisa klinička je remisija ili barem slaba aktivnost bolesti. Cilj fizikalne terapije je održavanje pokretljivosti zglobova i kralježnice, održavanje ukupne funkcionalne sposobnosti bolesnika, te sprječavanje propadanja mišićne mase zbog bolesti i neaktivnosti. Fizioterapeuti provode edukaciju i razvijaju program vježbanja za svakog pacijenta individualno. Pacijentu je važno naglasiti da ne smije štedjeti zglobove radi smanjenja problema.
Abstract (english) Psoriatic arthritis is a chronic inflammatory arthropathy that can affect the axial skeleton and peripheral joints. Psoriatic arthritis is characterized by monoarthritis, asymmetric oligoarthritis, irregular toe deviation, sausage swelling, absence of rheumatoid factor and subcutaneous nodules, often resorptive and destructive skeletons of hand and foot changes, spondylitis, sacroiliac joint and skin changes, and psoriatic joints. Psoriasis occurs significantly more often in people with arthritis than in the population. The prevalence of psoriasis has been shown to be 2-3% in the population. High familial aggregation of the disease clearly indicates the importance of a genetic factor, and more so. However, it should be said that not all people, carriers of genetic predisposition, always suffer from psoriatic arthritis. The disease most often occurs between the ages of 20 and 40. Physiotherapy assessment in psoriatic arthritis consists of a subjective examination, an objective examination, measurement and test procedures, and a treatment plan. The treatment of psoriatic arthritis is always combined and consists of general measures, general and local drug treatment, physical therapy and, if necessary, surgical treatment. The goal of treating psoriatic arthritis is clinical remission or at least low disease activity. The goal of physical therapy is to maintain the overall functional ability of the patient, the mobility of the joints and spine, and to prevent the deterioration of muscle mass due to inactivity and disease. Physiotherapists conduct education and develop an exercise program for each patient individually. It is important to emphasize to the patient that he must not spare the joints in order to reduce the problem.
