Abstract
Crijevna mikrobiota sastoji se od milijardi mikroba koji žive u strukturiranom ekosustavu kojim dominiraju obligatno i fakultativno anaerobne bakterije. U ljudi crijevna mikrobiota sudjeluje u različitim ulogama koje reguliraju fiziološku, metaboličku, nutritivnu i imunološku funkciju. Disbioza, promjena ili neravnoteža ovih bakterija, povezana je s ekstraintestinalnim i gastrointestinalnim bolestima. Transplantacija fekalne mikrobiote (FMT) je prijenos fecesa od zdravog darivatelja u debelo crijevo bolesnika čija je bolest uzrokovana promijenjenom mikrobiotom kako bi se obnovila normalna mikrobiota i tako izliječila bolest. Za FMT se koriste različiti putevi primjene pripravka koji uključuju gornje gastrointestinalne puteve tj. nazogastričnu/nazojejunalnu sondu, endoskopiju, oralne kapsule i donje gastrointestinalne puteve kao što su retencijska klizma, sigmoidoskopija ili najčešće kolonoskopija. S oko 95% uspješnosti, najučinkovitija i najbolje proučena indikacija za FMT je Clostridioides difficile infekcija. Prema preliminarnim studijama, FMT može imati i terapeutski potencijal za druge bolesti kao što su metabolički sindrom, upalna bolest crijeva, pretilost i funkcionalni gastrointestinalni poremećaji. Međutim, još uvijek nema dovoljno znanstvenih i kliničkih podataka koji bi podržali korištenje FMT-a za druge bolesti.
Abstract (english)
The intestinal microbiota consists of billions of microbes living in a structured ecosystem dominated by strict and facultative anaerobic bacteria. In humans, the intestinal microbiota participates in a variety of roles that regulate physiology, metabolic, nutritive, and immune function. Dysbiosis, a change or imbalance of these bacteria, is associated with extraintestinal and gastrointestinal diseases. Fecal microbiota transplantation (FMT) is the transfer of feces from a healthy donor to the colon of a patient whose disease is caused by an altered microbiome in order to restore the normal microbiota variety and thus treat the illness. Various routes of administration are used for FMT, including the upper gastrointestinal tract, ie the nasogastric / nasojejunal tube, endoscopy, oral capsules, and the lower gastrointestinal tract, such as retention enema, sigmoidoscopy, or most commonly colonoscopy. With about 95% success rate, the most effective and well-studied indication for FMT is Clostridioides difficile infection. According to preliminary studies, FMT may also have therapeutic potential for other diseases such metabolic syndrome, inflammatory bowel disease, obesity, and functional gastrointestinal disorders. However, there is not yet enough scientific or clinical data to support the use of FMT for other diseases.
