Objectives: The aim of this research was to investigate and analyze massive transfusion protocols in KBC Split during 2019 according to two criteria of massive transfusions and accordingly the consumption of blood products in specific units and for certain diagnoses and to collect epidemiologic, demographic and clinical data of the patients treated with mass transfusions. Participants and methods: Research had been conducted on 120 patients who fulfilled the conditions for receiving massive transfusions in KBC Split from 1st of January 2019 to 31st of December 2019. Inclusion criteria for participants were the receipts of at least 6 units of transfusion of red cell concentrates (criteria 1) and transfusion of at least 12 units of red cell concentrate over 24 hours (criteria 2). Participants were grouped according to the two aforementioned criteria and compared based on number, age and sex of participants, diagnosis, unit in which they were treated, count of transfused blood products and their ratios and survival rate. Data of participants involved in the study was collected from the information database “e-Delphyn” that is provided by the transfusion department of Republic of Croatia. Results: Research consisted of 120 participants with average age of 64 years of which the majority were men (63%). More participants fulfilled the conditions for massive transfusion for the 2nd criteria (70%) with a higher survival rate (86%) compared to patients that received massive transfusions according to the 1st criteria with a survival rate of only 50%. Highest consumption of blood products for massive transfusions was on the department of cardiac surgery (45%) and the most common diagnosis was polytrauma. Average ratios of blood products for the treatment of traumatic bleeding were in concordance with MTP while the average ratios of blood products for the treatment of non-trauma bleeding were not in concordance with the recommendations for massive transfusions. Conclusion: During 1 year, 120 patients were the recipients of massive transfusions in KBC Split. Non-traumatic bleedings required massive transfusions more often than traumatic bleedings. To ensure a more reliable approach, every hospital should have a defined guideline for massive transfusions, when to start it, how to monitor patients and provide recommended ratios of blood products so that hemostasis can be accomplished in patients with MT more quickly.
