Abstract
Anoreksija nervoza (AN) je poremećaj u jedenju s najvećom smrtnošću od svih psihijatrijskih poremećaja. Karakterizira je teški gubitak tjelesne mase zbog namjernog izgladnjivanja bez organskog uzroka. Najčešće se javlja u adolescenciji, a upravo je u ovoj populacijskoj skupini AN najčešća kronična bolest. Cilj istraživanja bio je utvrditi učestalost netiroidne bolesti štitnjače u populaciji pacijenata s AN Centra za poremećaje u jedenju djece i adolescenata Kliničkog bolničkog centra Sestre milosrdnice Zagreb. Opažajna retrospektivna studija obuhvatila je 183 djevojčice i adolescentice, prosječne dobi 14,8 ± 2,5 godina s dijagnozom AN u trajanju od prosječno 12,8 ± 13,9 mjeseci i prosječnim gubitkom na masi 21,7 ± 9,9 %. Netiroidna bolest štitnjače je utvrđena kod 62,6 % pacijentica, od čega je 73,5 % restriktivni tip AN. Spolni hormoni su značajno povezani s funkcijom tiroidne žlijezde (FSH r=0,407; LH r=0,468; estradiol r=0,282). Pacijentice kod kojih je zabilježen veći postotni gubitak tjelesne mase su imale manje vrijednosti T\(_{3}\) (r= – 0,352) i T\(_{4}\) (r= – 0,230). Promjena u funkciji tiroidne žlijezde je značajno povezana s kardiovaskularnim zdravljem (T\(_{4}\) i trigliceridi r=0,279). Rezultati ističu važnost hitnog tjelesnog oporavka s obzirom na brojne utvrđene metaboličke poremećaje i njihove potencijalne negativne doživotne posljedice u oboljelih od AN.
Abstract (english)
Anorexia nervosa (AN) is an eating disorder with the highest mortality of all psychiatric disorders. It is characterized by severe weight loss due to self-inflicted starvation in the absence of somatic cause. It is the third most common chronic disease in adolescents, who are at the highest risk of AN. The aim was to determine prevalence of non-thyroid disease among AN patients from at the Centre for eating disorders in children and adolescents in University Clinical Hospital Center „Sestre Milosrdnice“ Zagreb. Observational retrospective study included 183 girls, average age 14.8 ± 2.5 years, diagnosed with AN 12.8 ± 13.9 months and average weight loss of 21.7 ± 9.9 %. Non-thyroid disease was found in 62.6 % of AN patients, out of which 73,5 % in restrictive type AN. Sex hormones significantly correlated with thyroid function (FSH r=0.407; LH r=0.468; oestradiol r=0.282). Patients with a higher percentage of body weight loss had lower values of T(_{3}\) (r = – 0.352) and T(_{4}\) (r = – 0.230). Elevated thyroid function correlated with cardiovascular health (T(_{4}\) and triglycerides r=0.279). The results emphasize the need for urgent physical recovery because of the numerous underlying metabolic disorders and their potentially detrimental lifelong consequences in AN patients.
