Abstract Svrha ovog istraživanja bila je ispitati parodontni status bolesnika s ranim stadijem KLL-a i usporediti ga s parodontnim statusom zdravih ispitanika iste životne dobi, te analizirati vezu između parodontoloških i hematoloških parametara kod KLL bolesnika. U istraživanju je sudjelovalo 60 ispitanika podijeljenih u dvije skupine. Ispitna skupina obuhvaćala je 30 bolesnika s KLL-om (stadij Rai 0), dok je kontrolna skupina bila sastavljena od 30 zdravih ispitanika iste životne dobi. Kriteriji isključivanja ispitanika bili su: postojanje neke druge sustavne bolesti, provedena parodontna i/ili antibiotska terapija unutar zadnja 3 mjeseca, uzimanje lijekova koji mogu negativno utjecati na stanje parodonta. Ispitanicima s više od osam zuba (24 ispitanika ispitne skupine, 28 ispitanika kontrolne skupine) određen je parodontni status pomoću parodontoloških indeksa (API, PBI, PPD, REC, CAL). Hematološki parametri očitani su iz krvnih nalaza napravljenih na dan pregleda. Ispitanici s KLL-om imali su statistički značajno veće vrijednosti svih promatranih parodontoloških indeksa u odnosu na ispitanike kontrolne skupine, te značajno manji broj zubi po ispitaniku. Analiza podataka pokazala je da su ispitanici s KLL-om bili statistički značajno stariji od ispitanika kontrolne skupine (p=0,026). Međutim i nakon statističkog uklanjanja utjecaja dobi razlika je ostala značajna. Testovi korelacije parodontoloških indeksa i hematoloških nalaza bolesnika s KLL-om pokazali su da nije bilo statistički značajne povezanosti. Rezultati istraživanja pokazali su da su ispitanici s KLL-om imali značajno lošiji parodontni status u odnosu na zdrave ispitanike kontrolne skupine, ali nije dokazana veza između parodontoloških i hematoloških parametara.
Abstract (english) Objectives: The aim of this study was to assess periodontal conditions in patients with early stage CLL and to compare it with the periodontal status of age-matched healthy controls and also to analyze the relationship between periodontal and hematological parameters in CLL patients. Materials and Methods: The study was conducted on 60 subjects divided into two groups. The test group included 30 patients with CLL (stage Rai 0), while the control group included 30 systemically healthy age-matched subjects. Exclusion criteria for both groups were: presence of other systemic disease, history of treatment for periodontitis, use of antibiotics during the previous 3 months, use of medications which are known to be risk factors for periodontitis. The participants with at least eight teeth (24 participants from the test group, 28 participants from the control group) underwent a full mouth examination and periodontal condition was assessed using periodontal indices (API, PBI, PPD, REC, CAL). Medical data for CLL patients were collected from the patients’ records, while hematological data were obtained from the haemogram performed on the same day as the clinical examination. Results: Difference between groups was statistically significant for age, number of teeth and frequency of dental checkups (p<0.05). Patients with CLL had significant ly higher average values of periodontal indices (API 0.81±0.18; PBI 2.72±0.68; PPD 3.40±0.53; REC 1.95±0.87; CAL 4.37±0.80) compared to the control group (API 0.69±0.15; PBI 1.91±0.45; PPD 2.51±0.40; REC 0.99±0.54; CAL 3.00±0.58). The correlation coefficients between age and periodontal indices showed statistically significance between age and REC (r=0.357; p<0.01), and age and CAL (r=0.295; p<0.05). Age was not statistically significant covariate for CAL (F=2.205; p>0.05), only for REC (F=4.601; p<0.05). After the removal of the statistical effect of age, the difference in REC between CLL and control group remained statistically significant (F=19.732; p<0.01; eta2=0.287). Statistically significant association between periodontal and hematological parameters in CLL patients was not found (p>0.05). Conclusions: The results of this study showed that participants with CLL had worse periodontal status compared to the healthy subjects from the control group, but the relationship between periodontal and hematological parameters was not proved.
