Abstract
Selektivni mutizam, kao jednog od rijetkih anksioznih poremećaja, karakterizira konstanta selektivnost u govoru djeteta u određenim socijalnim situacijama. Kod djece prisutnost selektivnog mutizma predstavlja prepreku u razvijanju socijalnih kompetencija kroz interakciju s osobama u svom okruženju. Kako bi djeca sa selektivnim mutizmom, koja odbijaju verbalnu komunikaciju, lakše komunicirala sa okolinom radi zadovoljavanja vlastitih potreba i želja, koriste neverbalni način komunikacije kod kojeg nije prisutna tako visoka razina anksioznosti. Unatoč tome što glavni uzrok navedenog poremećaja još uvijek nije u potpunosti utvrđen postoje mnogi čimbenici koji utječu na njegovu pojavnost. Čimbenici se mogu podijeliti u dvije osnovne kategorije, a to su okolinski i biološki, pri čemu se na biološke čimbenike ne može utjecati zbog njihove već predodređene genetske podloge. Pod obiteljske čimbenike, kao jedne od vrlo važnih okolinskih čimbenika, između ostalog ubraja se i obiteljska povijest bolesti koja ima značajan utjecaj na psiho-socijalni razvoj djeteta. Kada se djeca suoče sa životnim događajem koji je za njih bio traumatičan, selektivni mutizam se u takvim situacijama javlja kao posljedica ili kao način na koji se djeca nose s osjećajima povezanim s traumom. Djetetov temperament utječe na interakcije roditelja i djeteta te može biti predispozicija za pojavu anksioznih poremećaja, isto tako i selektivnog mutizma. Unatoč tome što su čimbenici međusobno drugačiji, simptomi koji je javljaju kao posljedica imaju zajedničku karakteristiku, odnosno kod svakog djeteta sa selektivnim mutizmom javlja se osjećaj pretjerane anksioznosti. Kako neki od čimbenika također pripadaju anksioznim poremećajima, njihovom prisutnošću najčešće dolazi do komorbiditetnih stanja sa selektivnim mutizmom. Kada su uz selektivni mutizam prisutna komorbiditetna stanja, dijete pokazuje simptome oba poremećaja što otežava njihovo liječenje ili pravilno dijagnosticiranje. U današnje vrijeme postoje brojne metode liječenja među kojima se najuspješnijom pokazala kombinacija kognitivno bihevioralne terapije uz lijekove. Kroz psihoterapijske pristupe radi na se mijenjanju djetetovog reagiranja i nošenja sa socijalnim situacijama.
Abstract (english)
Selective mutism, as one of the anxiety disorders, is characterized by constant selectivity in child’s ability of speaking. Among children attendance of selective mutism represent an obstacle in developing social competency through interaction with people in their surroundings. Children with selective mutism, that refuse verbal communication, would communicate easier using nonverbal communication where level of present anxiety is not that high in way to satisfy their needs and desires. Even though the main cause of this disorder is not entirely established it is known that there are many factors that can effect appearance. Factors can be divided in two basic categories, such as environmental and biological factors, whereat on biological factors can not be affected because of theirs already predestined genetically base. In family factors, as one of very important environmental factors, is also included history of family disease which has been shown as very significant influence on child psycho-social development. When children face some kind of traumatic event in life, selective mutism can be appeared in this situation as outcome or as the way children bear with feeling associated with trauma. Child temperament affects parent’s interactions with child and can be predisposition for appearance of anxiety disorders, as well as selective mutism. Despite that the factors are mutually different the symptoms that appears have mutual characteristics, apropos at every child with selective mutism appears the feeling of excessive anxiety. Some factors also belong in anxiety disorders so with their appearance the final outcome is comorbid state with selective mutism. When there is presence both of selective mutism and comorbid state, child is showing symptoms of both disorders which makes difficult to treat them or make right diagnosis. Nowadays there are many methods of treatment and as a most successful has been shown combination of cognitive behavioral with medicaments. Through psychotherapy experts are trying to change child’s reactions and way of carrying with social situations.
