Abstract Pojavnost sindroma izgaranja prilikom pružanja palijativne skrbi kod medicinskih sestara u općoj bolnici
Uvod: Sestrinska profesija vrlo je kompleksna i odgovorna. Potreba za emocionalnim angažmanom i dugotrajna izloženost naporima odgovornog radnog mjesta može rezultirati pojavom posebne vrste stresne reakcije nazvane sindromom izgaranja na poslu. Kao najbrojnija skupina profesionalaca u sustavu zdravstva, sindrom izgaranja među medicinskim sestrama znatno je viši spram ostalih zdravstvenih djelatnika.
Cilj istraživanja: Cilj ovog istraživanja je ispitati pojavnost sindroma izgaranja kod medicinskih sestara koje pružaju palijativnu skrb u odnosu na one koje ne pružaju palijativnu skrb pacijentima, te ispitati utjecaj nekih osobnih čimbenika na pojavu sindroma izgaranja.
Ispitanici i metode: U ispitivanju je sudjelovalo 100 medicinskih sestara, od toga 50 ispitanica koje rade s palijativnim pacijentima i 50 ispitanica koje ne rade s palijativnim pacijentima. Ispitivanje je provedeno na bolničkim odjelima kirurške i internističke djelatnosti, u jedinici za intenzivno liječenje, na odjelu za pedijatriju i neonatologiju, te na odjelu za neurologiju. Za potrebe ovog istraživanja korišten je anonimni upitnik o općim sociodemografskim podatcima te upitnik o izgaranju na poslu autorice Christine Maslach.
Rezultati: U 46% ispitanica koje rade s palijativnim pacijentima utvrđena je visoka razina emocionalne iscrpljenosti u odnosu na 0% ispitanica koje ne rade s palijativnim pacijentima što predstavlja statistički značajnu razliku. Emocionalno su iscrpljenije najviše one sestre sa srednjom stručnom spremom, starosti između 36 i 45 godina i one starije od 56 godina. Medicinske sestre koje rade s palijativnim pacijentima pokazale su u 40% slučajeva umjerenu i visoku razinu depersonalizacije, dok su ispitanice koje ne rade s palijativnim pacijentima u 98% slučajeva pokazale nisku razinu depersonalizacije što također predstavlja statistički značajnu razliku. U ovom istraživanju odabrane sociodemografske karakteristike ispitanica nisu bile statistički značajne za procjenu depersonalizacije i osobnog postignuća.
Zaključak: U nedostatku adekvatno organizirane palijativne skrbi u općim bolnicama važno je naglasiti potrebu dodatne edukacije iz područja palijativne skrbi koja bi medicinskim
sestrama omogućila više znanja i vještina iz tog područja. Tada bi se lakše nosile s izazovima palijativne skrbi, više bi vrednovale svoj rad, a korisnici bi bili zadovoljniji pruženom palijativnom skrbi u akutnoj bolnici. Pružanje psihološke pomoći medicinskim sestrama koje rade s palijativnim pacijentima, koja je sastavni dio preventivnog programa u sprečavanju sindroma izgaranja u organiziranom sustavu palijativne skrbi, trebalo bi biti dostupno i sestrama koje provode palijativnu skrb na akutnim bolničkim odjelima.
Abstract (english) Incidence of burnout syndrome in nurses when providing palliative care in general hospital
Introduction: Nurse profession is very complex and responsible. The need for emotional engagement and long-term exposure to the workplace environment can result in the appearance of a special type of stress called burnout syndrome. Largest numbers of professionals in health care system are nurses so burnout syndrome among them is considerably higher than in the other health care professionals. Aim of the research: The aim of this study is to obtain date of incidence of burnout syndrome in nurses who provide palliative care compared to nurses who don´t provide palliative care. Furthermore, the aim of the study is to examine the impact of some personal factors on the incidence of burnout syndrome. Subjects and methods: 100 nurses participated in the study, 50 of them working with palliative patients and 50 of those not working with palliative patient. Study was conducted in general hospital on surgical and internal units, in the intensive care unit, furthermore, at the department of paediatrics and neonatology and department of neurology. The research was conducted using sociodemographic questionnaire and Maslach Burnout Inventory. Results: There is statistically significant difference, related to emotional exhaustion with 46% of the respondents who work with palliative patients having a high level of emotional exhaustion while nurses who don´t work with palliative patients don’t have high level of emotional exhaustion. Nurses with high school qualifications, age rang between 36 and 45 and older than 56 are mostly emotional exhausted. Palliative care nurses showed moderate or high level of depersonalization in 40% of cases, while nurses who don't work with palliative patients reported, in 98% of cases, low level of depersonalization which represent a statistically significant difference. Sociodemographic characteristics were not statistically significant for assessing depersonalization and personal accomplishment. Conclusion: In the absence of adequately organized palliative care in general hospitals, it is important to emphasize the need for additional education on the field of palliative care
which would provide the nurses with specific knowledge and skills in this area. This would result in constructive dealing with the challenges of palliative care, so they would value their work higher, and the patients would be more satisfied with the provided palliative care in the setting of acute hospitals. Psychological assistance should also be available to the palliative care nurses who work at acute hospital departments. That is an integral part of preventing burnout syndrome among them.
