Abstract Hipertenzija je najčešći povod za posjetu liječniku u općoj populaciji, te od nje u svijetu boluje oko 600 milijuna ljudi. Oko polovica oboljelih nema adekvatnu terapiju. To dovodi do razvoja komplikacija. Prema uzroku hipertenzija može biti primarna, ako je njen uzrok nepoznat, ili sekundarna, ako se uzrok može dokazati. Dijagnoza hipertenzije se postavlja kada je sistolički tlak barem 140 mmHg, a dijastolički barem 90 mmHg. Najčešća je renovaskularna hipertenzija koja označava aterosklerotski ili genetski promijenjene krvne žile bubrega. To uzrokuje poremećaj bubrežne funkcije i razvoj hipertenzije. Primarni aldosteronizam označava povećanje ili tumor nadbubrežnih žlijezda, što dovodi do pojačanog lučenja aldosterona i povišenja krvnog tlaka. „Sleep apnea“ nastaje kada tijekom spavanja meko nepce i jezik padnu na stražnju stijenku farinksa i suze dišni put. Dolazi do hrkanja, ali i hipoksije koja je uzrok hipertenzije i povećanog kardiovaskularnog rizika. Feokromocitom je uglavnom dobroćudni tumor nadbubrežne žlijezde koji uzrokuje hipertenziju uz preznojavanje, blijedilo ili crvenilo lica. Cushingov sindrom označava poremećaj lučenja hormona iz nadbubrežne žlijezde, što osim visokog tlaka dovodi i do promjena u izgledu pacijenta. Koarktacija aorte je prirođeni poremećaj koji se može otkriti u dječijoj dobi, a liječi se operativno. Uloga medicinske sestre važna je u edukaciji bolesnika o samoj bolesti, komplikacijama, te važnosti redovite primjene lijekova, kao i kontrole. Cilj edukacije je postizanje što veće prijemljivosti za terapijski režim i nužne promjene životnog stila. Terapijski režim uključuje nefarmakološke i farmakološke mjere.
Abstract (english) Hypertension is globaly the most common cause of medical examination. There are about 600 million people who have hypertension. Half of them don't have suitable therapy. That leads to development of cardiovascular complications. Hypertension can be primary, if the cause is not clear, or secondary, if it can be detected. Hypertension is diagnosed if the value of systolic blood pressure is at least 140 mmHg, and at least 90 mmHg for diastolic. Most common type of secondary hipertension is renovascular where blood vessels are atherosclerotic which disrupts kidney function and causes hypertension. Primary aldosteronism is caused by disruption adrenal glands function that leads to hypertension. „Sleep apnea“ happens during sleep when the soft palate and thoung fall on the back wall of oropharynx. That causes constriction of the airway, snoring, hypoxia and hypertension. Pheochromocytoma is mostly benign tumor of adrenal glands that leads to hypertension with sweating, pallor or redness of face. Cushing's syndrome leads to high blood pressure, but also changes of the appearance of the patient. Coarctation of aorta is congetial condition that can be detected in childhood, and the treatment is surgical. The nurse's role is important for the patinent's education about his disease, complications, and importance of using medications. It's goal is to achieve better receptivity for the treatment and lifestyle changes. Therapeutic regimen includes nonpharmacological (diet, physical activity) and phramacological measures (medicaments).
