Acute bronchiolitis is most commonly caused by RSV, and is characterized by symptoms of infection of upper respiratory tract leading to symptoms of lower respiratory tract infection that include wheezing and crackles. It typically occurs in children under two years of age. Patients are mostly treated at home, but few of them require hospitalization (6). Therapy of viral bronchiolitis is based on supportive care. Supportive care includes provision of oxygen, fluid management, aspiration and nasopharyngeal suction (2). Inhalation of hypertonic saline leads to temporary clincal improvement, so it is still used as a therapy (3). Administration of glucocorticoids is not indicated in the treatment of bronchiolitis because of the lack of scientific evidence of its effectiveness (2). The Cochrane report, that included 30 randomized controlled trials, concluded that treatment with salbutamol does not improve oxygen saturation, doesn't reduce hospital admission, doesnt't shorten the duration of hospitalization and doesn't reduce the time to resolution of illness (12). However, some studies have proved temporary clinical improvement so it can be used in children with severe bronchiolitis (6). Nebulized adrenaline, due to potential adverse effects and short-term effect, is usually not used in the home setting, but may be considered in children with severe bronchiolitis under medical supervision (5,18). Therapy of bronchiolitis with anticholinergic agents doesn't have scientific approvement (18).
